YOU ACKNOWLEDGE THAT OUR SERVICES ARE NOT INTENDED FOR EMERGENCY PURPOSES; IN THE EVENT OF A MEDICAL EMERGENCY, YOU AGREE TO IMMEDIATELY DIAL OR VISIT YOUR NEAREST HOSPITAL, OR DIAL 9-1-1.
PRACTICE POLICIES FOR PATIENTS
We want to help you and ensure that your experience at our medical practice is positive and stress-free. To ensure the best service, please read our practice policies carefully.
PATIENT SIGN-UP AND CONSENT FORMS
All patients are required to complete the Online Health Questionnaire. All required forms and documents must be completed before the appointment to ensure that our team has all the necessary information and can provide the best possible care. Please note that we reserve the right to cancel the appointment if the required forms and documents are incomplete. If you have any questions or concerns about the forms and documents required for your appointment, please do not hesitate to contact us.
VISITS
Consultations with your provider can occur in-person, over the phone, or through a telehealth video platform. Follow-up visits typically last between 30-60 minutes, with the frequency determined collaboratively by you and your provider.
Video Visits: When the provider is ready, we’ll email and text a link to join the video visit. You can use your phone, tablet, or computer.
SCHEDULING, CONFIRMATION & CANCELLATION OF APPOINTMENTS
At Beyond Medicine, we operate strictly on an appointment basis. If changes to your appointment are necessary, please inform us via email at [email protected] at least 48 business hours in advance. Be advised that appointments can be rescheduled once, are non-transferable, and non-refundable, with any deposit made credited towards the cost of your visit. Should we need to cancel your appointment from our end, a full refund will be issued promptly.
BLOODWORK & ADDITIONAL STUDIES
Many of the tests require a 10-hour fast. You can and should only drink water during this fast.
Costs of all testing will be reviewed with you by our staff after your medical consultation before labs are drawn. Once you receive final lab results, please e-mail all results to [email protected] in just ONE (1) e-mail (when possible) or upload results through the patient portal. You will be guided through the test results interpretation at your follow-up visits. Always send sensitive information over secure online platforms.
MESSAGING WITH YOUR CARE TEAM
We strive to be your healthcare partner, whether you’re seeking encouragement or have specific questions. Members can access messaging via designated chat for quick inquiries, support, or recommendations. If an issue is too complex to resolve through messaging, your provider may request a brief phone call for further discussion. Please expect a response within 24-48 business hours (if not before), as we are unavailable during nights, weekends, and holidays.
PRESCRIPTIONS AND PRESCRIPTION RE-FILLS
We refill the most common medications and can provide you with new prescriptions if medically warranted and safe. However, we are unable to e-prescribe controlled substances such as barbiturates, narcotics, benzodiazepines, or marijuana-containing products . We also cannot prescribe new prescriptions for higher-risk conditions like acute heart problems, transplant medications, ‘biologicals’, or hormone therapy (excluding birth control and other HRT). We can only prescribe medication only if it is medically appropriate.
MEDICAL RECORDS
Medical records can only be released with your authorization. You are responsible for obtaining previous medical records from other physicians or healthcare providers. A medical records release form will be provided for your use. Please contact your physician or other health care provider to obtain these records.
INSURANCE INFORMATION
BEYOND MEDICINE LLC does not accept health insurance. Consultations, evaluations, and concierge services must be paid for out-of-pocket. However, some (if not most) of the recommended testing and other diagnostics may be covered by insurance, potentially with a copay. Specialty tests (like stool, saliva, and some urine tests) may not be covered by most insurance plans.
Please note, we do not submit medical claims on your behalf, and we cannot assist you with claim resolution. If you have questions about your medical coverage, please contact your insurance provider using the phone number or member portal provided on your insurance card. Patients will be asked to sign a form prior to a visit that acknowledges understanding of this policy.
FEES AND UPGRADES
You can upgrade your package at any time until the day of your appointment. If you do so, you will only be charged the price difference. If you want to upgrade after that period, you will need to book a new consultation. Costs associated with laboratory tests, diagnostic studies, and pharmacy orders are separate from the fees of this office. Patients are responsible for any costs or co-pays incurred for labs and any other studies recommended by our Beyond Medicine physicians. The prices for some services may vary based on the complexity of your case and any services requested outside of regular hours. Fees are introductory and valid until the specified date.
FINANCIAL RESPONSIBILITY
You (“client”, “patient”) are solely responsible for all incurred charges, including consultation fees, tests, and any other associated costs during my visit. Payment is due on the day of service. If your appointment is within 24 hours, you will be charged 100% of the visit upon confirmation. All charges will be itemized and reviewed with you.
PAYMENT METHODS
Our office accepts cash, Venmo, ATH Movil, PayPal major credit cards (MasterCard, Visa, Discover) for services rendered.
PHONE CALLS, MESSAGES & FAXES
If you have a medical emergency, call 911 or go directly to the nearest emergency room immediately. Our Telemedicine Services are not for medical emergencies.
